Youth Service Award Application Form
Please Note - There are two opportunities to apply for consideration for an award per annum. All applications should be submitted in EITHER April/May OR October/November in any year. Interviews will be held in June and December, with results given following the completion of them ALL. Please note that applications at other times of the year cannot be considered.

Applicants will receive an e-mail acknowledging receipt of their application, and within 14 days you will be notified if you will be required to attend for interview.

An award will not be made without an applicant attending an interview.

Not all applicants who are interviewed will receive an award.
If selected for interview you will be asked for documentation to support your application.
