• Licence for Occupation of Aves Housing a Registered Provider Hostel, Specified Exempt Accommodation HB Regulation 75H Reference HB Circular A8/2014  & Service Charge Schedule

    Aves Housing (RP Reg. No. 4664), whose office is at 17 Bensham Rd Thornton Heath CR7 7AD (“The Landlord”) and the Licensee (Client Name) with Hostel address from the Licence Start Date to the Licence End Date.  This Licence to Occupy a Room in the Hostel replaces any previous agreement you have entered into for occupation at this Hostel. Move on accommodation is signposted not provided. Your assigned room is subject to change during the term of the licence and as such will NOT appear on this document.

    1 The Landlord permits the Licensee to occupy a Room in the hostel, occupation being by The Licensee only, and to use the Furniture & Furnishings, food. At the end of the term, this licence will continue on a periodic basis.  Facilities for preparing food will be provided and we will enter your room from time to time to check your wellbeing. 

    2 The Licensee shall pay to The Landlord: The amount in the Rent Breakdown Below. The Licensee is liable for all the gas, electric, water to the utility suppliers.

    3. You have been referred to us by a government department or a homeless organisation as requiring support to maintain your tenancy. As a Registered Provider (4664) in accordance with the Regulator of Social Housing, we offer as part of the tenancy and provided by us your landlord, Support & Supervision and will provide facilities for preparing food adequate to your needs.

    4. I give full authorisation my Local Council to share any and ALL information regarding my HB claim Appeals or Tribunals with representatives of Aves Housing and that for client HB purposes I confirm that I will keep the DWP and the council fully informed about any change of circumstance that could affect my benefits and that I currently reside at the (Hostel).  I confirm that this is my main and only residence.

    1. Obtain a Photo of the Client and Upload to Client Photo

    2. For UC clients obtain a good copy of the last UC Payment Breakdown. A photo from a phone may not be adequate. This may have been completed in the Viewing & Client Assessment form earlier. If so ignore this step

    Do not issue a licence for a client with zero UC income or UC that has not been processed

    3. For UC clients, complete a UC change of address on their Journal by first logging in and on the Home Page. This may have been completed on the Viewing & Client Assessment form earlier. if so ignore this step

    Select Report a Change in Circumstances

    Select Where you live and What it Costs

    Select Change then I have moved and then enter the Post Code

    Select the Correct Address

    Complete the Day you Moved from the Licence Start Date

    Select No Housing Costs because the council will pay the rent

    Submit this form

    Go to The Journal and take a copy the last journal entry which will be Where you live and what it costs. 

    Ensure that the name of the client is visible at the top right of the screen

    Upload a PDF of this with no spaces in the filename to UC Change of Address proof.

     

    4. Making an HB Claim

    Is the Client moving to a Merton Hostel or the Client moving to a  new borough? - Make a new HB Claim then save a copy until you have added all the evidence then submit and upload a PDF copy of the submitted claim to this form

    Is the Client moving into Croydon? - Make and submit an HB Claim then Email the evidence to croyhben@croydon.gov.uk and upload the email to this form

    Is the Client moving within a borough but not Merton? - It is ok to fill an online form or COA email to the council with a new licence and upload a copy of the form or Email to this form

    5. Print this Licence, ask your client to Sign it at the Hostel

    6. Give your client the keys and verify that they can Open the Doors and complete the Client Induction

    7. Make a PDF of the signed licence with no spaces in the filename and Upload this to "PDF of Signed Licence".

    8. Complete the HB Claim that you started earlier or the COA Email or Form

     If you have made a new HB application, make a PDF of the FULL CLAIM FORM. The Claim Summary is not adequate on its own as it will not have evidence of the answers to all the questions on the form such as giving them authority to provide information to us or backdating (this will be much more work later if there is a dispute and you may need to submit another HB claim and apply for backdating and it may be necessary to appeal the desision). Upload this to "PDF of HB Application". 

    9. Once complete select make a New HB Record, Submit and Submit the HB Manager form when it opens. Do not make multiple HB Records.

    Ineligible Service Charges are things like personal gas, electric and water and are not part of HB.

  •  Details of the Tenancy Related Support Offered

    You will be offered support around (but not limited to) the following areas:  

    A risk and needs assessment & support plan that is regularly reviewed.  Supervision in your project at least once a month. Contact at least every 14 days.  Help to claim and manage housing and other benefits, fulfilling tenancy conditions and developing housing management skills.  Assistance with Maintaining the Health and Safety of the Building through monthly onsite meetings.  Assisting with arranging minor repairs to equipment and appliances.  Advising service users on the use/maintenance of domestic equipment and appliances.  Arranging adaptations in connection with the tenant’s disability.  Advising and assisting with Housing related matters, i.e. rent, Benefit claims, antisocial behaviour issues and neighbour and tenancy disputes etc.  Assistance with Budgeting Advice and Assistance.    Signposting and keeping appointments at GP, dentist, optician services, Substance Misuse & Mental Health.  Tenancy support including budgeting for move on and assistance to obtain ID, open a bank or Credit Union account.

    Life skills development such as advice & guidance, filling in forms,  helping you develop social skills through house meetings and group interaction and support with moving on into independent accommodation and job searching.  Dietary Advice is given.  General wellbeing- reconnecting family, joining a gym, religious services.  Assisting with employment, and help with organising eventual move on to independent accommodation.  You will also be able to get involved with the organisation including involvement on our RSL recognised Tenant Panel.  Failure to engage with these services can result in the ending of the licence.

    A 24-hour concierge service. The service operates 365 days per year. This service provides for onsite concierge staff and a rapid response team to assist any emergency issues that require preventative action or a quick resolution to protect the property and maintain tenancies. 

    Hostel Rules 

    The Licensee has read and agrees to the terms of this licence and will abide by the conditions of the Hostel Rules including reference to: Any illegal activity.  Antisocial behaviour.  Loud music.  Guests past 9 pm. Smoking in the hostel. Failure to recycle waste.  Failure to join a Credit Union. Not saving for your move on.  Adding or removing or Beds Furniture or cluttering your room.  If we suspect illegal activity in the Hostel we reserve the right to search your room in your presence.  Any of the above may result in an immediate termination of your licence and disposal of any items left in your room after 48hrs. 

    Service Charges: You and the other occupants of the hostel are responsible for the utilities as agreed in the pre-tenancy document. Aves Housing will not be responsible for any Gas Electric or Water bills for your hostel. Housing Benefit does not include such payments and Aves Housing is strictly forbidden according to DWP Rules from contributing from Housing Benefit payments.
