• Aves Housing (RP Reg. No. 4664), whose office is at 17 Bensham Rd Thornton Heath CR7 7AD (“The Landlord”) and the Licensee (Client Name) and address from the Licence Start Date to the Licence End Date.  This Licence to Occupy, replaces any previous agreement you have entered into for occupation at this address. Move on accommodation is signposted not provided. Your assigned accommodation is subject to change during the term of the licence and as such will NOT appear on this document.

    1. The Landlord permits the Licensee to occupy a Room in the hostel, occupation being by The Licensee only, and to use the Furniture & Furnishings. At the end of the term, this licence will continue on a periodic basis.  Facilities for preparing food will be provided and we will enter your room from time to time to check your wellbeing. 

    2 The Licensee shall pay to The Landlord the amount in the Rent Breakdown. The Licensee is liable for all the utilities, gas, electricity, water and you consent to your information being shared with the utility suppliers.

    3. Aves Housing offer Supported and Non-Supported accommodation. UC Housing Costs clients are provided with Non-Supported accommodation. Housing Benefit clients are provided with Care, Support or Supervision part of the tenancy in accordance with Specified Exempt Accommodation HB Regulation 75H Reference HB Circular A8/2014.

    4. I give full authorisation my Local Council to share any and ALL information regarding my HB claim Appeals or Tribunals with representatives of Aves Housing and that for client HB purposes I confirm that I will keep the DWP and the council fully informed about any change of circumstance that could affect my benefits and that I currently reside at the address. I confirm that this is my main and only residence. 

    5. I agree to abide by the House Rules including allowing access to Aves Housing staff www.aveshousing.org/hostel-rules and for Housing Benefit clients I agree  to engage with the Tenancy related support offered www.aveshousing.org/services/support

    1. Obtain a Photo of the Client and Upload to Client Photo

    2. For UC clients obtain a good copy of the last UC Payment Breakdown. A photo from a phone may not be adequate. This may have been completed in the Viewing & Client Assessment form earlier. If so ignore this step

    Do not issue a licence for a client with zero UC income or UC that has not been processed

    3. For UC clients, first Log in to UC, then complete a UC change of address on their Journal by first  and on the Home Page. This may have been completed on the Viewing & Client Assessment form earlier. if so ignore this step

    Select Report a Change in Circumstances

    Select Where you live and What it Costs

    Select Change then I have moved and then enter the Post Code

    Select the Correct Address

    Complete the Day you Moved from the Licence Start Date

    Select No Housing Costs because the council will pay the rent

    Select I live in supported accommodation

    Submit this form

    Go to The Journal and take a copy the last journal entry which will be Where you live and what it costs. 

    Make a note in the Journal that I have move to supported accommodation with Aves Housing (4664) they will claim Housing Benefit from the council. This is why there are no housing costs. 

    Ensure that the name of the client is visible at the top right of the screen

    Upload a PDF of this with no spaces in the filename to UC Change of Address proof.

    4. Making an HB Claim by following the detailed notes below depending on the borough.  

    5. Print this Licence, ask your client to Sign it at the Hostel

    6. Give your client the keys and verify that they can Open the Doors and complete the Client Induction

    7. Make a PDF of the signed licence with no spaces in the filename and Upload this to "PDF of Signed Licence".

    8. Complete the HB Claim that you started earlier or the COA Email or Form

     If you have made a new HB application, make a PDF of the FULL CLAIM FORM. The Claim Summary is not adequate on its own as it will not have evidence of the answers to all the questions on the form such as giving them authority to provide information to us or backdating (this will be much more work later if there is a dispute and you may need to submit another HB claim and apply for backdating and it may be necessary to appeal the decision). Upload this to "PDF of HB Application". 

    9. Once complete select make a New HB Record, Submit and Submit the HB Manager form when it opens. Do not make changes to this form or re-submit after an HB Record has been made or you will make duplicate HB Records.

    10. Ending the Licence. If a client wishes to end the licence and vacate the room. Obtain the date the client wishes to end the license. Make a note in the contact record. If possible obtain evidence. This can be and WhatsApp photo or an email. Update the CR and the HB notes and change the HB status to past once the room is cleared. Do not issue an SOS as the person is giving up the room voluntarily.

     

    UC - Housing Costs

    If your client is receiving UC and moving to Lewisham or Bromley then you will need to login to UC and report a change of circumstances - click in where you live and what it costs,  change and enter post code and sellect the correct addresss then enter the date the licence starts and select  housing costs (YES).

    10. Moving in

    Once the ID, NINO, UC change of address, and proof of income have been uploaded, the HB application has been filled in completely and saved for later. Then it's time for you to arrange to meet your new customer at the property.

    Make sure that the staff member arrives 30mins before to check that the keys work and that the property is in a clean state. Clean if required. Arrange for a utility top-up if required and complete the induction. Request a google review if the process has been completed correctly.

  • For Borough of Croydon 

    The client is moving into the borough? - Make a New HB claim. You will not be able to upload claim evidence (ID, Licence, Income) to this form. Save the form before you submit it so you have a copy of your Full Answers. Check the PDF of the downloaded HB claim by opening it to make sure it's ok. Then submit the form and save the confirmation, again as a PDF. Upload both the full claim and confirmation PDFs to this Record.

    The client is moving from our hostel in Croydon?  - don't make a New HB new claim.

    In both cases, because you cannot add documentation to the online form, send an email to croyhben@croydon.gov.uk with the ID, Signed license, Proof of income (usually UC payment as a PDF), UC change of address (again as a PDF) AND the Supported Exempt Accommodation) SEA document. Save this email as a PDF and add to this record

    Ensure you write good notes indicating you have send email with SEA so that these notes can be seen when HB record is opened.

    The link above will bring you to Welcome to better off Enfield.  Click on "Access and track" in red under your money to "Use the calculator to find out what benefits you are entitled to". Sign for your client or, if your client does not already have an Enfield Connected account make on with Create an Account (Click Register). 

    Once you have registered the client will get a confirmation email from Enfield.  Get them to follow the instructions on this email to complete the registration. Then Sign in to Enfield Connected using the email and password you created to start the benefits calculator.  Complete this and then complete the HB Application BUT DO NOT submit until you have uploaded all documents including ID, Licence, UC COA. Save it before you submit and upload to this form. Once all documents have been uploaded save again and then submit and save. We save many times because sometimes the council's websites do not let you save a copy of the answers after you have submitted them.

    You will be offered Care around (but not limited to) the following:

    Guiding and assisting a person with a disability as they go about tasks that would be relatively simple for a person without such a disability: buying food, preparing food etc.  Carrying out tasks on behalf of a disabled person because they do not have the physical or mental capacity to do those things at all, not even with supervision: dealing with correspondence, arranging appointments etc.   Life skills development such as advice & guidance, filling in forms,  helping you develop social skills through house meetings and group interaction and support with moving on into independent accommodation and job searching. Dietary Advice is given. 

    You will be offered Tenancy Support around (but not limited to) the following:  

    A risk,needs assessment & support plan that is regularly reviewed.  Help to claim and manage housing and other benefits, fulfilling tenancy conditions and develop housing management skills. Assisting with arranging minor repairs to equipment and appliances. Assistance with Maintaining the Health & Safety of the Building. Advise on the use/maintenance of domestic equipment and appliances.  Arranging adaptations in connection with the your disability.  Advising and assisting with Housing related matters, i.e. rent, Benefit claims, antisocial behaviour issues and neighbour and tenancy disputes etc.  Assistance with Budgeting Advice and Assistance.   Signposting and keeping appointments at GP, dentist, optician services, Substance Misuse & Mental Health.  Tenancy Sustainment support includes budgeting for move on and assistance to obtain ID, open a bank or Credit Union account.   General wellbeing- reconnecting family, joining a gym, religious services.  Assisting with employment, and help with organising eventual move on to independent accommodation.  You will also be able to get involved with the organisation including involvement on our RSL recognised Tenant Panel.  Failure to engage with these services can result in the ending of the licence.

    You will be offered Supervision around (but not limited to the following):

    Supervision in your project at least once a month.  A minimum of 3 hours direct Contact at least every 7 days.  monthly onsite meetings.   A 24-hour concierge service. The service operates 365 days per year. This service provides for onsite concierge staff and a rapid response team to assist any emergency issues that require preventative action or a quick resolution to protect the property and maintain tenancies. 

    Hostel Rules 

    The Licensee has read and agrees to the terms of this licence and will abide by the conditions of the Hostel Rules including reference to: Any illegal activity.  Antisocial behaviour.  Loud music.  Guests past 9 pm. Smoking in the hostel. Failure to recycle waste.  Failure to join a Credit Union. Not saving for your move on.  Adding or removing or Beds Furniture or cluttering your room.  If we suspect illegal activity in the Hostel we reserve the right to search your room in your presence.  Any of the above may result in an immediate termination of your licence and disposal of any items left in your room after 48hrs. 

    Service Charges: You and the other occupants of the hostel are responsible for the utilities as agreed in the pre-tenancy document. Aves Housing will not be responsible for any Gas Electric or Water bills for your hostel. Housing Benefit does not include such payments and Aves Housing is strictly forbidden according to DWP Rules from contributing from Housing Benefit payments.
