  • Saving Orphaned Souls Sheila's Animal Rescue Mission ( SOS ARMS) is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. SOS ARMS is completely funded through fundraising efforts and public donations. All of our pets are cared for in our two shelters, or in foster homes or in private pension and receive complete medical care, which includes worming, vaccinations, spaying/neutering and microchipping prior to their adoption. This questionnaire is designed to help SOS ARMS and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. SOS ARMS reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions! Please be informed that the there will be an adoption Fee up to £400 that covers : full vetting ( all dogs are travell ready prepaired for entering the UK according to DEFRA's requirements : passport, full vaccination , deworming , neutering if the animal is over 6 months old and tests for different diseases) ***NOTE : If the dog you are adopting is under the age of 6 months, neutering will be a clause of the Adoption Agreement and your responsibility . We will ask for a vet prove when the age will allow to have the neutering surgery *** For dogs with special needs a full description and medical record will be provided for the future care. • Transport fee . Fees are payable via PayPal or wire transfer and must be paid in advance of transport for out of area adopters.

    Sumitting this application does not mean you are commiting to a dog, nor does it guarantee you will get that dog. It is simply a tool we use to help both you and us identify a good potential match for your family and lifestyle. A successful adoption depends on selecting the right dog for your household and a good understanding of the dog's needs and care requirements.   • We will be speaking to all household residents to confirm everyone is aware of this application process and wants to have a dog added to the family. Please do not submit this application if you are "trying to talk someone into" adopting this dog or think "once they meet the dog, they will want one" or "I'm getting this dog as a birthday present for them." In those scenarios, you are welcome to email us to discuss the dog, but we would like all adults in the home to be involved in that email discussion.
