Personal information should not be shared on this form. All responses will be held securely and may be shared with 3rd parties for the purpose of raising awareness on the issues around Fume Events and Aerotoxic Syndrome in the campaign for safer commercial air travel. If you are Cabin Crew or Flight Crew and were on duty you must also report any incidents through your own Company reporting system.
