  • Part 2 - Player Medical Information

  • Please ensure that all relevant medical information is available to the Band Management in order that we can look after your child appropriately. If any medical information changes, please let the Band Manager know as soon as possible in order that we can keep our records up to date.

    Please note that for legal and safety reasons we require that all medication (except inhalers) for players under 16 years be handed to the Band Manager when on trips.

  • Is the player up to date with tetanus immunisation?

  • Is the player up to date with other immunisations recommended for the UK? (eg meningococcal C, mumps, measles and rubella)

  • Does the player suffer from any of the following listed conditions? If other than "none", please give details in the free text box at the end of this section.

  • Does the player suffer from any other medical condition requiring medical treatment, including medication? If yes, please click yes below and give details in the free text box at the end of this section.

  • Is your child allergic to any medication, insect bites or food? If yes, please give details in the free text box at the end of this section.

  • The player is in good health and able to participate in activities with Youth Brass 2000.

    I give permission for proprietary medicines to be administered (as outlined above above) as required.

    In the event of an emergency, I agree to the player being given any emergancy medical, surgical, optical or dental treatment as considered necessary by the medical authorities present.

    I will notify the band officers of any changes in the player's circumstances.

    I will ensure that the player has adequate supplies of medication where required.

    I agree that when away on trips, the Band Management may act in loco parentis in emergencies.

    Should any medical or accident situation occur, if a player's parent/guardian is not present, the Band Management undertake to endeavour to to contact them on an urgent basis. 

  • Part 3 - Photographic Permission

  • We regularly use photographs and videos of the band and its players for publicity, and on our website. Whilst it is not feasible to remove individuals from full band photographs and videos (official or unofficial), we do sometimes use small group photographs, and we have a “players’ gallery” on the website. We never use players' full names in any photographs. If you are happy for the player to appear in these photographs, please select "yes" in the box below.

  • Part 4 - Player Code of Conduct

  • As a member of Youth Brass 2000, high standards of behaviour & conduct are expected of you at all times. This Code of Conduct has been drawn up in order to protect the good name of Youth Brass 2000, & to ensure that all members of the band are kept safe. The leadership & committee reserve the right to either suspend transgressors from any event, or to leave the band. Instances of misconduct may be reported to parents / guardians of the band member(s) concerned.


    Code of Conduct

     Instructions given by Youth Brass 2000 leadership must be followed without delay or argument.

     Youth Brass 2000 contains players of varying age groups. Different directives may be given according to age group – eg bedtimes when staying overnight – these directives must be adhered to without question.

     Different physical boundaries may be set at events depending upon age group. These must be adhered to without question.

     Alcohol – under-age drinking is not permitted. Alcohol is not allowed in bedrooms under any circumstances.

     Smoking – smoking is not permitted at any time at any band event.

     Drugs – No player may take, receive or possess any illegal substance. The band leadership must be advised of the use of prescribed drugs for medical purposes.

     Conduct (behaviour & language) – high standards are expected of you at all times. Youth Brass 2000 is a highly regarded band with an enviable reputation in the eyes of the public. Player(s) indulging in conduct which is likely to damage this reputation will be dealt with at the discretion of the band leadership.

     Punctuality – players must notify the band leadership of their availability or changes to their availability (via Teamstuff, e-mail or phone call) in advance. Players must aim to ensure they are punctual for rehearsals and events.
