To make sure that the sometimes vulnerable people (Patients, Children, etc) our staff work with are fully protected, we have to ask you to sign the declaration below. You can speak to us privately if you feel you may not be able to make the declaration, nothing that you tell us is written down or recorded in anyway whatsoever. If you have a criminal record, you may still be able to work with us, but not with groups who are considered vulnerable. You do not need to give a reason for not signing this declaration if you do not want to. If you declare you have no criminal record and we send you to work with vulnerable people, we will check your file with the Criminal Records Bureau to ensure you have no record. A copy of the information we receive will be sent to you for you to keep. The copy which we receive will be kept securely by us for the duration of your assignment or for six months (whichever is the shorter). By signing below you are also giving us permission to check any records that the police or authorities may hold in your name.
