  • Dear Area Service Director for Youth Involvement and Activities,

    Here is a charter application for a new Y's Youth Club. I am aware that it is my responsibility as the submitting officer to make sure that all the minimum charter application requirements are met and that you do not have the authority to endorse an application unless they are met.

    I have carefully scrutinised these application papers and hereby submit this application.

  • Club Member Roster

  • All new applications require a typed membership roster, in a format that can be uploaded to the IHQ database.

    You must download and complete the Roster Template before proceeding with this application.

    All mandatory fields within the template (marked in red) must be completed:

    • Club Position
    • Courtesy Title
    • First Name
    • Last Name
    • Gender
    • Birthday
    • Address (at least one - home, work, or both)
    • Preferred address type
    • Phone number (at least one - home, work, mobile or all)
    • Preferred phone type
    • E-mail address

    Your application may not be processed if this template is altered in any way or if any required information has been ommitted.

     

    NOTE: Club President and Club Secretary must both have a valid email address. The email addresses of remaining club members is recommended and necessary to allow them access to their profile on the database system.

  • NOTE: All members who join the club within a 60-day period immediately following the date of receipt of the complete charter application in the International Office (which date becomes the official “charter application date”) will be accepted as charter members. All such names must be added to a new, blank copy of the roster template and forwarded to the International Office by e-mail before the termination date.
