  • Thank you for choosing to participate in an OYS event. In order to ensure we comply with national and international standards, guidelines and policies, we need you to complete the following form before you will be able to participate in the event.

    If you have a disability that may make the completion of this form difficult, the form can be completed by someone on your behalf, however we will still need your signature. If you have any trouble completing the form or have a query please contact lourdes@oblateyouthservice.com or call Padraig Corcoran on 087 772 3255. The form should only take about 10-15 minutes to complete in one go.

    Throughout the form, you will see fields (boxes where you type the information) marked with an asterisk (*). These fields are mandatory and you will be unable to progess on to the next page until you complete the field.

    In order to continue to comply with best child and vulnerable adult safeguarding practices and policies, we need to get a profile photo of you for our records, just like your student or work ID. Your image will be kept securely for our records only and will not be released to third parties.

  • One of the key strengths of the OYS is the diverse nature of our membership. We have people from all walks of life.   • European Health Insurance Card (EHIC)

    The EHIC is vital to receiving medical attention abroad should the need arise. They are available for free at www.ehic.com.

  • Consent for Medical Treatment

    If it becomes necessary for me to receive medical treatment, I hereby give my full consent to any necessary medical treatment, and authorise the designated leader to sign any document required by the hospital authorities. I undertake to inform the designated leader if any of the information given on this form changes before an event takes place.

    In consideration for the Oblate Youth Service, I hereby agree to indemnify the Oblate Youth Service, its volunteers and staff against all claims, costs, actions and demands whatsoever resulting from taking part in the programme of activities or the administration of medicines, unless such claims, costs, actions or demands result out of the negligence of designated Oblate Youth Service volunteers or staff.

  • Consent for Media

    I consent to photographs, videos and other media being used as publicity material for the Oblate Youth Service and/or the Oblates of Mary Immaculate.

  • The two documents that are linked below are the two Child and Vulnerable Adult Protection (Safeguarding) policies that govern OYS activities at home and in Lourdes. You must familiarise yourself with both documents before you can progress.

  • Vetting and Child & Vulnerable Adult Protection - Over 18

    As part of the OYS Child & Vulnerable Adult Protection Policy, you are required to submit to a relevant national vetting process (where applicable).

    You must read, complete and consent to the general declaration and terms. In all cases the welfare of the child and/or vulnerable adult is paramount.

    Because there are a number of jurisdictions with different processes, the following pages are region-specific.

  • Vetting: Republic of Ireland

  • The national vetting process in the Republic of Ireland is provided by An Garda Siochana's Central Vetting Unit. This is administered at a Diocesan level by the Dublin Archdiocese Child Safeguarding & Protection Service. We liaise with these groups on your behalf. Once your vetting has been cleared, this lasts for 3 years.

    If you are under 18 years of age, an appropriate parental consent form will also accompany the vetting application.

    Padraig Corcoran is the Vetting Coordinator for the Republic of Ireland - any queries regarding the process should be directed to him (lourdes@oblateyouthservice.com).

    You will not be able to become an OYS member (and therefore will be prohibited from taking part in activities, events and trips) unless you consent to this process.

    If you are a current OYS member who has been vetted by us recently, please just click "Yes" below and we will advise you when you need to renew your vetting (if applicable).

    In order to facilitate a smooth and secure processing of your vetting application, once you consent to vetting below, you will receive the paperwork in the post and the Vetting Coordinator will be in contact with you.

  • Vetting: Northern Ireland

  • The national vetting process in Northern Ireland is provided by Access NI. This is administered by an umbrella company (Security & Vetting Solutions Ltd.). We liaise with these groups on your behalf. Once your vetting has been cleared, this lasts for 3 years.

    Two vetting clearance certificates will be issued. One to you and another to the OYS.

    If you are under 18 years of age, an appropriate parental consent form will also accompany the vetting application.

    You will not be able to become an OYS member (and therefore will be prohibited from taking part in activities, events and trips) unless you consent to this process.

  • The national vetting process in England and Wales is provided by way of a CRB check. This is administered by National Criminal Records Bureau. We liaise with these groups on your behalf. Once your vetting has been cleared, this generally lasts for 3 years.

    If you are under 18 years of age, this vetting process may not be applicable to you. The vetting coordinator named below will be in contact with you in any case.

    Ronan Lavery is the Vetting Coordinator for England and Wales - any queries regarding the process should be directed to him (lavery.ronan@googlemail.com).

    You will not be able to become an OYS member (and therefore will be prohibited from taking part in activities, events and trips) unless you consent to this process.

  • Vetting: Scotland

  • The national vetting process in Scotland is provided by way of the PVG Scheme (Protecting Vulnerable Groups). This is administered by the local safeguarding team in your local Oblate parish. We liaise with these groups on your behalf. Once your vetting has been cleared, this lasts indefinitely.

    Once you have been cleared, you will be issued with a certificate from PVG. You may also be required to attend a parish safeguarding workshop to complete the process.

  • Code of Behaviour and Conduct - Over 18

    • The OYS uniform is to be worn for all pilgrimage activities.
    • OYS members will be suitably and appropriately attired at all other times.
    • OYS members will present themselves for work on time and in fit condition to undertake assigned duties.
    • No alcohol shall be consumed during the hours of work under ANY circumstances.
    • OYS members under 18 years of age will not be permitted to consume alcohol for the duration of the trip except with the prior written consent of a parent/guardian.
    • Hours of work will be assigned by the Director in consultation with the pilgrimage section leaders.
    • The reputation and integrity of the OYS shall be upheld and promoted at all times by all members.
    • Disciplinary proceedings will be brought against any OYS member not in compliance with the above or with the expressed wish of the Director/pilgrimage section leaders.
    • All OYS members will be directed on a daily basis by their group co-ordinators and any concerns or issues must be brought to forward to the co-ordinators.

  • As part of the OYS Child and Vulnerable Adult Protection policy, a number of new measures have been implemented.

    Congratulations, you completed your Character Reference requirements last year, so no further action is required

     

    Character Reference - Former OYS Member

    Please give names, addresses and telephone numbers of two people we may contact who have known you well for at least two years, and would be able to comment on your suitability for this role.

    We cannot accept references from your relatives or family members. WE CAN ACCEPT references from priests or deacons, or members of your Parish Safeguarding Team.

    Please only select people who know you.

     

    Character Reference - New OYS Member

    Please give names, addresses and telephone numbers of two people we may contact who have known you well for at least two years, and would be able to comment on your suitability for this role.

    We cannot accept references from your relatives or family members. WE CAN ACCEPT references from priests or deacons, or members of your Parish Safeguarding Team.

    If you are a new member who is still in school, it is OK to add two teachers and/or the headmaster/mistress. The general school address, email address and phone number is fine - you don't need to provide the personal contact details for the teachers.
