If you have been issued with a HPA ID card this must be returned to HPA, Chantry House, 22 Upperton Road, Eastbourne, East Sussex, BN21 1BF
Once the process is started you will lose your verified status online. Inline with the terms and conditions of registration your basic profile may remain visible to the public for up to 36 months on the registry and be reflected as expired. Any cautions, suspensions or struck off notices will remain in place for this period. Please note that even after deregistration is complete if HPA receives verified evidence of misconduct, criminal behaviour or unsafe practice covered by the HPA code of conduct regarding yourself as a clinician during your time on the registry you will still be subject to standard investiation by the fitness to practice committee and an advisory notice may be added to your public profile.

Once processed you may re-apply to the HPA Council register after 12 months have passed since your de-registration date.
