We’re sorry to hear that you wish to end your registration with HPAC. Thank you for your time with us as a Registrant — we value your contribution and wish you every success for the future.
To process your request securely, please complete the deregistration form in full after reading the important information below.
Deregistration During Investigation or with Active Conditions
Deregistration does not prevent an ongoing investigation, nor remove you from the register if you are subject to any active caution, suspension, or condition. If HPAC receives credible evidence of misconduct, criminal behaviour, or unsafe practice relating to your time on the register, you may still be investigated by the Fitness to Practise Team.
Where an outcome results in a caution, suspension, or Removed from Register, your profile will be reinstated on the register for public safety, with the relevant status displayed.

Re-application
You may re-apply to the HPAC register after 12 months from your deregistration date.
- Struck-off notices remain in effect permanently.
- If deregistered during an investigation, re-application is not permitted for five years.

Important: HPAC ID Card
Please remember that your HPAC ID card may not be used in any capacity if you have deregistered, to do so may constitute fraud by deception and may result in your profile being reinstated and converted to "Removed from Register" and a referral being made to Police and DBS.
