Registration for Quit Smoking CPD session, ONLINE via Zoom
This form is for you to register your interest in a cpd. It does not put you under any obligation. The cost for 'Quit Smoking' is £50.

I also offer 'Control Your Drinking' for just £25 when you book for 'Quit Smoking'. If you complete this form, you can let me know later if you would like to add 'Control Your Drinking' and I will tell you more about it.

The dates are not set in stone. Please talk to me if the times and dates on offer do not work for you. In order to give you plenty of time for questions, finishing times are not exact.
