What information we collect?
Swansea University is the ‘Controller’ of the personal data you provide to us. The University has a duty to ensure that you have a clear opportunity to disclose information relating to your disability and any specific requirements you may have, in a way which will not affect your treatment. Information pertaining to an individual’s disability is defined as special categories of personal data (sensitive). The University is committed to protecting the rights of students and student data under current Data Protection legislation. Why we collect this information?
The information we collect via this form will enable us to make reasonable adjustments so your College and potentially other services e.g. Residences, Occupational Health within the University of any adjustments that need to be made.
How long we keep it?
Any information concerning your disability / specific need / medical condition provided to the Disability Office will be held securely for 7 years and thereafter will be destroyed.
Who we may share your data with?
To make an effective response to your disclosure the Disability Office may need to communicate disclosed information to appropriate members of staff for example those involved in teaching, accommodation, libraries, attendance monitoring etc.
The Disability Office may pass disclosed information to a third party e.g student finance Wales, student finance England, NHS, and other funding bodies for the purpose of supporting a student with applying for funding, answering queries or supporting a student in relation to their application
In exceptional circumstances the Disability Office may also pass disclosed information to a third party (e.g Fitness to practice) without consent where there is believed to be serious risk to the health and safety of yourself and/or others.
