WHY ENTER?
The winning pharmacy will be one in which every member of the team has played a vital role in delivering a first-class service to the local patients, and which will also be able to clearly demonstrate the benefits of collaborative team working.
With continued talk about GPs’ workload and healthcare, stretched to its full capacity, the capacity for community pharmacy to clinical care cannot be overemphasised. To optimise this contribution, multidisciplinary team working opens up access to primary care and helps to reduce the pressure on other healthcare services. As such, collaboration between different healthcare professions is vitally important.
Regardless of whether that collaboration is with GPs, practice-based pharmacists, district nurses, physiotherapy teams or other healthcare professionals, it is the working in partnership that leads to optimal healthcare outcomes.
The Integrated Care Partnerships, which came out of the Transforming Your Care initiative, have already shown the value of collaborative work between healthcare professionals from across various sectors as they develop integrated care pathways in areas such as diabetes and stroke.
This award is designed to recognise pharmacy teams which have worked in partnership with other healthcare professionals, regardless of the strand of healthcare involved.
WHO CAN ENTER?
This category is open to all pharmacy teams - either independent or chain - which can clearly demonstrate a successful track record of working closely in collaboration with other healthcare team.
Nominations for the category can be made through self-nomination, by colleagues or pharmacy business owners, or by personal application.
All entries will be judged by an independent panel of experts.
JUDGES WILL BE LOOKING FOR:
• Pharmacy teams which have not only teamwork, but also collaboration at the heart of their business, with staff members who actively encourage and support each other, and who seek opportunities outside of the pharmacy walls to improve patient care.
• Teamwork that is evident in the examples put forward of good practice, which clearly highlight the benefits to patients in the local community.
• Clearly defined, well-evidenced examples of initiatives and projects, which not only showcase the collaborative nature of the work involved, but which clearly illustrate the vital role played by team collaboration and integrated working.
