TRIP INFORMATION FORM

Please complete this form in full should any of the following events take place:

Any travel or activity outside of Ottawa without the host or the school;
Any overnight travel or overnight activity with the host.

This form, duly filled out, must be submitted at least 7 days prior to the event taking place, or as soon as scheduled, if this happens within such 7 days. Go Canada will then ask for authorization from the student’s parents given the information provided in this form. Once authorization has been received, Go Canada will confirm to you via email, at which point, the trip and/or activity will be deemed authorized.
