In the event that the third party product fails the Initial Assessment, the fee will not be invoiced.

  • Product Information
  • Please provide information about the product to be assessed. Additional information such as Product Brochures, Installation, Administration and User Guides can be attached at the end of this form.
  • Product Installation & Configuration
  • Please provide as much information as possible on the installation and configuration of the product to be assessed.


  • Please provide supporting information regarding how and where the product is to be installed. This should include physical access, device access, rights and permissions need to install the item and the method of installation (physical on-premise installation, remote software delivery)
  • Please indicate who will be responsible for the installation of the product e.g. the third party or the school. Please also indicate if you require either Capita or C2k assistance with the installation and configuration of the product.
  • Please include information on any required physical access required to undertake the installation and configuration as well as any requirements for physical network ports, space, power and cooling.
  • Product Support
  • Please provide as much information as possible on the ongoing maintenance support of the product including how calls for support are to be initiated. Please indicate who will be responsible for the ongoing support of the product e.g. the third party or the school. This should include frequency and management of patches/upgrades, physical access, device access, rights, and permissions need to install the item. Please also indicate if you require either Capita or C2k assistance with the support of the product.
  • Active Directory Requirements
  • Active Directory Service Accounts will only be granted at the lowest level of account privileges required for the item being assessed to be installed or operate. Please provide sufficient level of information to allow the assessment to be accurately undertaken.
  • Networking & Communication Requirements
  • The data network is strictly controlled with multiple layers of network security and filtering in operation. It is therefore very important that you accurately describe the network requirements and operation of the item to be assessed. Failure to do so may result in the request for assessment being rejected or the unexpected operation of the item when in service.

  • User Provisioning and Authentication
  •  
  • Some applications can benefit from the population of user data from existing MIS systems and can facilitate Single Sign On through existing credentials. Please indicate if the item to be assessed requires the use of either functionality and provide details of how it is to be implemented including if provisioning is expected to be dynamic or to provide an initial seed only.Note: A Data Sharing Agreement may be require to be obtained from the individual school.
  • Systems Integration
  • In addition to any requirements for User Provisioning and Single Sign On, please describe any other integrations required by the product. This should include requirements for items such as, but not limited to print functionality, data transfer or exchange, mail relay, integration with other existing School or C2k systems.
  • Backup and Restore Requirements
  • It is assumed that any backup/restore requirements are contained within the product being assessed and are the responsibility of the requestor or the school. Please describe the requirements for the backup/restore of the 3rd party product including the backup/restore policy, backup media type, location of stored backups, retention period etc.
  • Remote Access

  • Remote Access is provided on a discretionary and individual school basis only. Where remote access is required, the individual School must complete and submit a signed "Request for Remote Access" form before remote access can be considered. The completed request is then subject to a further review process prior to any remote access being granted.

  • Some software applications allow executable code to be developed and executed. Such applications are strictly controlled within the C2k Network.
