Thank you for applying to RSY-Netzer Shemesh 5779. This is the Enrolment Form. Please complete it in a single sitting, if you do not do so, your data may be lost. The form should take no more than 30 minutes to complete and you will have the chance to review your application at the end.
Page 1 - RSY-Netzer's 'Brit Kehilla' (Code of conduct for young people) - please read through this with the participant.
Page 2 - Participant's Details - This is where you can enter the personal details of the participant applying to be on camp. You will also be asked to upload a passport style photo of the participant (jpg., jpeg., or png. no larger than 100kB).
Page 3 - Parent/Guardian Details - Here you can enter the contact information of the participants parent/guardian(s). Parent/guardian 1 is the primary contact. This person will be responsible for settling payment and receiving event information.
Page 4 - Dorm Sharing - This is where the participant can place up to 5 names (in no particluar order) of participants they wish to share a dorm with and whether there is anyone they cannot share a dorm with.
Page 5 - Welfare and Medication and Dietary Requirements - Here you will be able to enter the medical and welfare details of the participant who will be coming on camp. Please have the date of your child's last anti-tetanus injection/booster, along with their doctor's and emergency contacts details. Additionally, RSY-Netzer runs a vegetarian camp with no fish, in line with our ideological commitment to Jewish ethical eating. You will have the opportunity to enter any dietary requirements of the individual.
Page 7 - Payment - Card payment will be required (by secure payment) and you can express whether you will require financial assistance. The deposit cost to pay on submission of the form is £330.
Once the enrolment form and payment of a deposit has been processed, you will receive an invoice for the rest of your payment with further instruction on how to pay. Your place on camp will only be confirmed once we have your form, and full payment is made.
The information you provide us with will be kept in the strictest of confidence. For your child’s benefit, it is helpful that even sensitive information is included as it will allow us to support your child during Shemesh. We are able to accommodate many special needs but withholding medical information could lead to your child being withdrawn from the event.
